Fees

Psychotherapy/counseling:                   $150 to $180 per 55-minute session (sliding scale)

Group therapy/counseling:                    $35 to $50 per two-hour session (depending on                                                                                             group size)

Therapy and training workshops:         $90 to $150 per 4 to 8-hour workshops

Depositions or courtroom testimony:   $300 for the first hour; $75 per subsequent 1/4 hour

Clinical evidentiary summaries for legal, social security and VA issues:  $150 per hour

Missed appointments: A fee of the hourly rate for the scheduled service will be charged for appointments cancelled less than 24 hours in advance. Exceptions will be made in the event of extenuating medical, personal or family circumstances.

PAYMENT PLANS:                       In some cases, to accommodate financial hardship, partial payment can be arranged temporarily with agreed-upon limits to the balance-owed. Unless otherwise agreed upon in writing with Dr. Maggard, payment is due at the time of service.

PAYMENT METHODS:               Cash, check, credit/debit card (Visa, MasterCard, Am Ex, Discover), Health Savings Account cards.

INSURANCE ACCEPTED:           Medicare;  Anthem BC/BS (including Federal Employee Program),  Humana,  Tricare West, Tricare East,  UnitedHealthcare, Optum.    If Dr. Maggard is not a provider for your insurance company, he will assist you at no charge in filing claims for any out-of-network reimbursement you may be due.
